Training goals

This training is aimed towards lead investigators conducting patient safety incident investigations and investigators supporting safety incident investigations.

This training is to support the development of systems-based safety incident investigations; in line with NHS guidance, based upon internationally recognised good practice.

Method of training

This training is limited to a maximum of 8 delegates and can be delivered in a one-to-one coaching session. This course can be delivered virtually, face to face, or it can be delivered in a blend of both formats.

 The course will provide interactive opportunities with trainers to practice the tools and techniques which build trust and skills which are necessary in systems-based investigations.

Training is reinforced with a simulated investigation.

Who will deliver the training?

Two trainers, both with extensive experience of conducting Independent NHS England Investigations into serious patient safety events:

Responsible Officer with over 35 years of clinical experience

NHS Nurse Consultant with over 35 years of clinical experience

When this course is provided as coaching of an individual, it will be delivered by one trainer.
